| Fax Anonymous Registry (FAR) Project for Electrical IncidentsThis project permits the anonymous reporting of electrical incidents by fax so that the incidents may be documented in a registry for research, innovation and education purposes.The project has evolved since 1997 when it started with Dr. Mary Capelli-Schellpfeffer during her time on the faculty of the University of Chicago Electrical Trauma Research Program. After a trial period in 1997, national distribution of the project's incident reporting form was promoted through the 1998 IEEE Petroleum and Chemical Industry's Safety Committee Paper submission by Danny Liggett, H. Landis Floyd, Kim Eastwood and Dr. Mary titled "How Can We Better Learn from Electrical Accidents". Subsequently, in the Summer of 1999, EC&M Magazine provided the report form to its readers, reaching an audience of electrical contractors, maintenance and electrical instrumentation professionals. If you have observed an electrical incident, consider reporting it anonymously. If you are an electrical accident survivor please consider reporting the incident which resulted in your injury.
